Documentation typically required

Where applicable, please include the following with your referral. Not all documents will apply to every referral type.

  • Medical reports and relevant test results
  • Independent medical expert reports (if applicable)
  • Current or last job description
  • Sick leave records (if available)
  • Work history / CV
  • Copy of ID document (where applicable)
  • Signed consent (where applicable)
  • Any additional collateral information relevant to the referral

Including medical reports, job description and relevant collateral documentation at the time of referral helps to expedite scheduling and ensures the assessment is as comprehensive as possible.
